ACTION AND MECHANISM Broad-spectrum antiseptic, antibacterial (bactericidal), antifungal (fungicidal), antiviral, antiprotozoal, and sporicidal. It is a molecular complex of iodine and povidone. The complex, as such, is inactive until the iodine, which is responsible for the antiseptic action, is released. Dilution increases the rate of iodine release. Iodine acts through oxidation-reduction reactions, altering many biologically important molecules, such as glucose, starch, glycols, lipids, and proteins. In this process, iodine is transformed into iodide, which is biologically inactive. Iodine has a very broad antimicrobial spectrum. No resistant microbial strains have been detected. CONTRAINDICATIONS - [IODIDE ALLERGY].- Do not use compounds containing mercurial derivatives.- Iodine loses activity in the presence of organic matter. PREGNANCY Animal safety: no data available. Safety in humans: In a recent study, use in low birth weight infants resulted in neonatal hypothyroidism. It crosses the placenta. Adequate and well-controlled studies in humans are not available. Its use is not recommended. Effects on fertility: no specific studies have been conducted in humans. INDICATIONS - Dermal solution, ointment, gel: Antiseptic and disinfectant for general skin use, minor wounds and superficial cuts, minor burns, bedsores. In the hospital setting as an antiseptic for the surgical field, puncture sites, wounds, burns, and surgical materials. - Scrub: Disinfectant wash for hands and skin. - Shampoo: Disinfectant wash for hairy areas of the body. POSOLOGY Depending on the indication, it can be used neat or diluted. - Dermal solution: After washing and drying, apply directly to the affected area. - Gel: After washing and drying, apply directly to the affected area, 1-3 times daily. After application, it is advisable to cover the area with gauze. - Scrub: Use a small amount of product, rubbing for 2-5 minutes until a lather forms. Rinse thoroughly with water or with a sterile gauze pad soaked in water. - Shampoo: Apply directly to the affected area. For the scalp, make two successive applications after rinsing the head with warm water. In the second application, rub until a lather forms and leave on for 5 minutes. Rinse thoroughly with water. PRECAUTIONS - Thyroid function tests should be performed in cases of prolonged treatment. - Avoid contact with eyes, ears, and mucous membranes. - Organic matter (proteins, blood, etc.) decreases its activity. - It is inactivated by sodium thiosulfate: a possible antidote in case of poisoning. ADVERSE REACTIONS Rarely, local irritation and hypersensitivity reactions may occur. Application of povidone-iodine to extensive wounds may produce adverse systemic effects such as metabolic acidosis, hypernatremia, and impaired renal function. OVERDOSE - Symptoms: Local: skin irritation. Treatment: Local: wash the affected area with plenty of water and apply anti-inflammatories topically, including corticosteroids. Accidental ingestion of large quantities: symptomatic and supportive treatment, monitoring acid-base balance and renal and thyroid function.
