Tris-EDTA (ethylenediaminetetraacetic acid) has bactericidal activity on non-growing bacteria located in biofilms ( Ciofu et al, 2017 ); in combination with tromethamine (Tris) it damages bacterial cell walls and increases penetration into bacterial membranes; it is well tolerated and non-ototoxic; it acts synergistically with chlorhexidine ( Guardabassi et al, 2010 ). Recently, an in vitro study demonstrated that Tris-EDTA is a more than effective adjuvant in the treatment of chronic cases of otitis media caused by Pseudomonas with the presence of potential biofilms, combined with antibiotics ( Pye et al, 2014 ). NAC is an N-acetylated derivative of the amino acid cysteine. It has mucolytic properties, thereby inhibiting the production of exopolysaccharides, thereby weakening the EPS matrix. It has recently been used in eight clinical trials as an adjunctive treatment to destroy mature biofilm and prevent its formation and has demonstrated an excellent safety and efficacy profile. The combination of Tris-EDTA and NAC on the skin and ear before the application of topical products such as chlorhexidine, antibiotics and synthetic antimicrobial peptides therefore allows an increase in the permeability of the mature biofilm and, consequently, its disintegration, effectively contributing to overcoming the problem of resistance to classical antibacterial therapeutic approaches.
